You must meet the following criteria to be eligible for services:
* Reside in Rockingham County
* Have no form of health insurance including Medicaid
* At least one qualified member of the immediate family must be gainfully employed, or receiving
unemployment, disability or Social Security retirement benefits
* Gross household income must fall at or below 200% of the Federal Poverty Guidelines
Other Requirements
Medicare recipients may be eligible for dental services only if all other criteria are met.

How to Become a Patient
Call for an eligibility screening appointment. Walk-ins are not accepted.
Applicants must bring the following items for eligibility screening:
2010 complete federal income tax returns (no recap sheets) with w-2's
Current proof of address (electric bill, phone bill, etc.)
Photo identification
Verification of all sources of income that apply to you and your spouse where applicable
one month's worth of current, consecutive check stubs (salary/wages)
unemployment benefit history
child support award letter
social security award letter
SSI award letter
bank statement showing direct deposit of social security, retirement, disability alimony
checks
any veteran service benefits
Workman's Comp
Work First
Patients are re-screened for eligibility every 6 months.
New patients will not receive a medical or dental appointment on the same day as their eligibility screening.
Clinic Fees
Our Clinic has the following fee requirements:
We are a private, non-profit organization. Since we receive no government assistance, our funding comes entirely from private donations and charitable trusts. We are not an entitlement program. A $10.00 administrative fee is requested for each medical and dental visit so that the Clinic doors can remain open to you and your community neighbors.

Staffing
The Clinic is staffed mostly by healthcare and lay volunteers, and our services are only possible through their kindness and dedication to working free of charge in the Clinic. Please extend a kind word or thank you to our volunteers for their gifts of time and expertise.
Patient Responsibilities
Please bring all of your medications with you to all of your medical appointments.
Notify Clinic of Changes
Please immediately notify our receptionist of changes in phone number, address, job, or insurance status for you or any other qualified member of your household.
FCRC Patient Expectations:
Be cooperative in following the guidelines set forth by the Clinic.
Be courteous and appreciative of the volunteers, staff, and community partners for
their gift of time to help you.
Be patient realizing that many did not get the opportunity to be served.
Be honest and forthright in your dealings with us.
Be accepting of the limitations that FCRC experiences.
- Be kind and courteous to others in the waiting area and/or on clinic grounds.
Canceling Appointments
Please be considerate of the services the Clinic is providing. Cancellations must be made at least 24 hours before the scheduled appointment. This allows us to give your appointment to someone else who needs it.
Be responsible. If you say you’re coming, COME. The Clinic’s “No Show” Policy is as follows:
- 1st No Show: Warning
- 2nd No Show: 30 day suspension from all services (including prescriptions)
- 3rd No Show: Permanent termination of patient privileges
